Sub: Application for five days’ sick leave

Respected Sir,

I am suffering from malaria and have been admitted to a hospital for treatment. The doctor has advised me to take complete rest and remain under medical care for at least five days. Even after that, I may need a few more days to regain my strength.

I kindly request you to grant me five days’ sick leave. I hope to recover within this period and resume my classes. If my recovery takes longer than expected, I will inform the school accordingly.
